Van Lennep Kliniek obtains personal data from you, for example the information you provide via email, intake form, website, social media, phone, app, text message, mail and/or oral. In addition, personal data can be obtained via third parties in the context of professional activities or services.

• Social Security Number
• Data important for carrying out the treatment, e.g. (medical) data.

Purposes

Van Lennep Kliniek processes personal data in the context of its medical services and the professional activities of a doctor and skin therapist. In addition, personal data is processed to serve the financial interests and/or commercial interests of Van Lennep Kliniek.

Foundations

Van Lennep Kliniek processes this personal data on the following grounds:

• Carrying out the treatment on your behalf;
• Compliance with legal obligations such as identification under the guidelines of the Health Care Inspectorate. (YOU)
• Management of the electronic health record;
• Invoicing and, if necessary, collecting funds, whether or not through collection measures;

Retention period

Van Lennep Kliniek will not keep your personal data longer than necessary for the purposes it has stated in this privacy statement. In this context, it is relevant for you to know that the retention period for professional liability is 20 years, so that the personal data you provide can also be stored for a maximum of 20 years.

• We store and use your personal data so that we can provide you with good care.
• We will only share your data with third parties if this is necessary to provide good care. If specific consent is required by law, we will request it from you in advance.
• We process your data on the basis of the treatment agreement as described in the Medical Treatment Agreement Act (WGBO).
• You can view parts of your personal data. Please send us a request (see bottom of this page).
• If you believe that the information we have stored about you is incorrect, you may ask us in writing to change your information.
• You can ask us to erase your personal data. If we do this, we (may) no longer be able to provide you with responsible care. The WGBO requires a retention period of 20 years. That is why, in that case, we store your data in an inactive archive, invisible and unusable by the normal user.
• You can withdraw your consent to our processing of your personal data. We may then (possibly) not be able to guarantee responsible care. In that case, we store your data in an inactive archive (see above).
• You can object to us if you do not agree with how we process your personal data.
• We will notify you if something has gone wrong with your personal data.
• We do not process your data longer than necessary to provide good care. We comply with the WGBO retention periods.

All of the above also applies to personal data that we receive from third parties.
In case of treatment, additional agreements will be signed.

We are transparent — your rights

You have the right to request Van Lennep Kliniek to provide access to your personal data. After receiving your request for access, you will receive an overview of your personal data processed and/or stored by us within a reasonable period of time.

To request this personal data, you can send an email to drs. L.J. Holst, after which we will initiate the procedure. You also have the right to object to the processing or storage of your personal data. This includes the right to request the rectification or deletion of your personal data. You can also request that your personal data be transferred to you. In addition, it is good to note that invoking the aforementioned rights may be rejected if processing your personal data is in the interest of correctly carrying out the assignment given to us, because of the doctor's professional secrecy or the storage of your personal data is necessary due to the legal requirements that apply to this.

If you see reason to do so, you finally have the right to file a complaint with the Data Protection Authority.
